Case Management
The Avicenna Case Management Team (or Social Work Team) is an interdisciplinary team of students and with the goal of connecting Avicenna patients with resources in Champaign-Urbana to meet patients’ health and social needs.
We connect patients to researched and established resources and agencies in Champaign-Urbana to address:
Obtaining medical insurance
Preventive and Diagnostic Medical Screening
Mental Health
Housing Insecurity
Food Insecurity
Other needs indicated by the provider and/or the patient
How can you get into contact with our team?
We conduct routine social assessments with patients directly after their first provider visit, and as needed as indicated by the provider thereafter. We also can meet with patients upon request if a patient needs assistance navigating social programs or would like to know more about their options.

Meet Our Leadership Team
Ella
Ella is a Junior at the University of Illinois at Urbana-Champaign. She has been part of the Case Management team since Fall 2023. Ella loves meeting patients and is passionate about providing accessible health care to the CU community.
Noah
Noah is a 4th year medical student at Carle Illinois College of Medicine. He has been a part of Avicenna Case Management since Spring 2024. Noah is pursuing a career in surgery and is passionate about providing patients with health care access in his community.
Ashwathi
Ashwathi is a Junior at the University of Illinois at Urbana-Champaign. She has been part of the Case Management team since Fall 2023. Ashwathi loves connecting patients to resources embedded in the community and being involved with the CU area at large.
Nayanika
Nayanika is a Junior at the University of Illinois at Urbana-Champaign. She has been part of the Case Management team since Spring 2024. Nayanika is passionate about connecting patients in the C-U area to resources that benefit their individual needs.
